Aniridi

Descripción

El iris es la estructura de color que se encuentra debajo de la córnea y cuyo orificio central constituye la pupila. El color del iris da la tonalidad de nuestros ojos. Es responsable en parte de regular la cantidad de luz que debe entrar en el ojo. Realiza la función de diafragma del ojo. Presenta un músculo de disposición circular que permite modificar el tamaño de la pupila. El término aniridia hace mención a la falta del iris del ojo, pero en realidad se encuentran involucradas más estructuras. Suele afectar a ambos ojos y no es completa, hay un iris incipiente que no ha llegado a desarrollarse. Es importante que la sociedad conozca la enfermedad para evitar el rechazo social que se da en muchas ocasiones. En los colegios hay que enseñar a los niños que su compañero es totalmente normal, explicarles porqué tienen que utilizar gafas de sol, los problemas que puede tener para la práctica de determinados deportes, el porqué de su mirada especial, etc

Causas

Existen dos tipos de aniridia, la hereditaria y la esporádica, en la que no existen antecedentes familiares de la enfermedad, pero que una vez adquirida se convierte en hereditaria. El patrón hereditario es autosómico dominante (la transmite el portador al 50% de su descendencia). Hay una falta de desarrollo del globo ocular durante el embarazo debido a una mutación genética. Se encuentra alterado el brazo corto del par 13 del cromosoma 11 y falta de proteínas en el ADN en el gen Pax 6, responsable de la formación el ojo. Debido a esto se encuentran alteradas varias estructuras del ojo e, incluso, otros órganos del cuerpo. Las familias afectadas deberían someterse a un estudio genético.

Epidemiología

Es una enfermedad de baja incidencia; se da aproximadamente 1 caso de cada 80.000-100.000 nacimientos.

Síntomas
El síntoma fundamental es la fotofobia (intolerancia anormal para la luz).
Hay baja agudeza visual, entre un 20 – 10%, que se agrava en el caso que la aniridia se acompañe de otras lesiones oculares como catarata o glaucoma congénito.
Dificultad para la visión lejana, por lo que para ver los objetos con nitidez tiene que acercarse mucho a ellos.
No distingue los detalles de un objeto a contraluz.
Para leer necesita luz directa sobre el papel y que no existan sombras ni reflejos sobre cristales o espejos.

Alteraciones asociadas
Nistagmus (movimientos involuntarios del ojo que impide fijar la visión).
Cataratas
Glaucoma.
Degeneración corneal.
Hipoplasia macular y del nervio óptico.
Estrabismo
Ambliopía: ojo "vago", se utiliza más el ojo de mejor visión. Se trata mediante parches, etc.
Luxación del cristalino.
Ojo seco.
Tumor de Wilms; consiste en un tumor en el riñón que se acompaña en ocasiones de retraso mental y alteraciones genitourinarias. Es importante realizar un diagnóstico precoz de la enfermedad para que no se extienda a otras partes el cuerpo y, en este caso, la supervivencia es alta.
Ataxia cerebelar.

Diagnóstico

El niño recién nacido cierra los ojos cuando hay luz y se encuentra más cómodo en la penumbra. Ante la postura del niño, debe ser revisado por un oftalmólogo para determinar el grado de afección y las posibles alteraciones asociadas. Es importante realizar chequeos periódicamente para descartar posibles alteraciones renales o de otras zonas el organismo. Se debe visitar el oftalmólogo cada tres o seis meses durante los primeros años de vida y posteriormente al menos una vez al año.

Tratamiento

En la actualidad no hay un tratamiento efectivo para la aniridia, tratándose sólo las alteraciones asociadas. La aniridia se padece desde el nacimiento, por lo que es conveniente que el niño sea consciente del problema desde el principio y de una manera progresiva. Tiene dificultad para leer, distinguir determinados objetos y para realizar determinadas actividades.

Hay que hacerle comprender que como tiene la pupila muy grande le hace más daño la luz que a otros niños y es necesario el uso de gafas de sol y otras ayudas visuales para realizar determinadas actividades. No es conveniente dramatizar la situación y procurar que haga una vida totalmente normal, como el resto de sus compañeros y amigos. Es aconsejable la asistencia a un centro escolar donde con ayudas visuales y la cooperación del profesorado pueda realizar exactamente las mismas actividades que el resto de sus compañeros y que la integración sea total. La organización ONCE dispone de centros donde pueden cursar estudios.

Las lentillas cosméticas con iris artificial pigmentado alivian la fotofobia y disminuyen el nistagmus (movimientos involuntarios del globo ocular), aunque no siempre mejoran la visión. Las lentes convencionales no suelen mejorar la agudeza visual por lo que tienen que utilizar lentes de baja visión.

Las ayudas visuales que se pueden considerar:
Gafas – lupa
Atril.
Flexo con lupa.
Libros adaptados con letra grande.
Programas específicos de ordenador.

Aniridia

Definición:

Es la falta del iris aunque en realidad están involucradas varias estructuras del ojo. Generalmente es bilateral e incompleta, pues en la mayoría de los casos existe un iris incipiente que no ha llegado a desarrollarse.

La apariencia externa del ojo anirídico es de una gran pupila central negra y en muchos casos una pequeña franja coloreada.

Prevalencia en España:

Es una enfermedad de escasa incidencia: 1 caso cada 80.000-100.000 nacimientos.

Síntomas:
•Fotofobia intensa
•Baja agudeza visual.
•A veces cataratas y glaucoma congénito.
•Nistagmus: movimiento involuntario de los ojos de origen neurológico.
•Degeneración corneal.

Tratamiento:
•En la actualidad no hay un tratamiento efectivo para la aniridia.
•Se deben utilizar ayudas ópticas..
•Vitrectomías.
•Tratamiento antiangiongénico

Limitaciones provocadas por la enfermedad:
•Dificultad para la visión lejana
•Falta de nitidez en la visión.
•Falta de contraste.

Ayudas:
•Lentes de contacto cosméticas con el iris pigmentado.
•Para la realización de tareas de cerca: lupa flexo, macrotipos, libros con letras grandes y otras ayudas ópticas.
•Control del deslumbramiento y de la fotofobia tanto en interiores como en exteriores: filtros especiales con o sin graduación.

I had to go to the hospital recently to have my eyesight checked. I sat in the waiting room and I struck up a conversation with 78-year-old Peter who’d undergone a corneal graft 10 years ago and comes in for annual checkups. My PhD focused on treating difficult to heal wounds by using different phases and concentrations of silver nanoparticles. Whilst firmly chained to the lab during my research, I got the opportunity to see my fair share of yucky wounds and because of that, nothing medically freaks me out. Because what he told me was horrific. Interesting but really gross.

I had loads of tests done at the hospital. The field view test, the Snellen Chart, a couple of scans to check for fluid build up and I had my eye pressure checked too for a signs of glaucoma. That was a strange one. Rather than the usual puff of air they give you at the opticians, I had some numbing drops put in each eye then, without much warning, a probe touched the middle of each pupil. ‘You’d have never let me do that without them’, the nurse told me. Feeling your eyeball squish backwards is an unusual feeling – not one I’d recommend for the squeamish. I also was asked by every nurse if I’d already had my pupils dilated because apparently…ahem… I have the biggest pupils they’d ever seen. I bet they tell that to all the girls.


After (top) and before (bottom) the dilating drops were added. Both pictures were taken in *very* bright light for comparison.

Finally, as you can see, I had the dilating drops. These stung a bit and made the world a peculiar place for a few hours. I couldn’t focus on anything due to the relaxation of my lens and because my massive pupils were letting in loads of light, everything was painfully bright. So as I talked to a vague shape of a doctor, I was told the good news – there’s nothing wrong with my sight. He then told me something pretty amazing. I have 6/4 vision. I’d heard of 20/20 but no idea what 6/4 meant so he explained it to me, unhelpfully with diagrams that I couldn’t focus on.

Think back to the Snellen Chart with the letters of varying sizes to measure visual acuity, measured as a fraction (chart below). The numerator (top number) refers to the distance you stand away from the chart which is fixed at 20 feet. The denominator (bottom number) indicates the distance at which someone with normal eyesight could read the bottom line of the chart. Normal vision is therefore 20/20. If you’re 20/40 then the line you can correctly read could be read by a person with normal vision at 40 feet and it goes all the way up to 20/200 and after that you’re registered as blind.
Switching to metric, the equivalent of 20/20 is 6/6 (6 metres = 20 feet). Being 6/4 (actually 6/3.8) means I’m 20/12.5 on the old scale, the line marked 10 on the chart above. 6/3 is the best human vision can get (the bottom line on the chart, number 11). It’s rather exciting to think I’m only 80 cm from ocular perfection!

What is 6/6 Vision?



6/6 vision is considered "normal"vision within the population. 6/6 vision means that at a distance of 6 meters, you should be able to read the 6 meter line on the Snellen vision testing chart (see below). If you can only see the 12meter line on the chart which contains larger size letters or numbers, and which normal people can see when standing 12 meters away, your vision is 6/12 ie below average. In other words the upper figure 6 is the distance you are form the chart, and the lower figure 12 is the lowest line you can read.



Similarly the 60 meter line on the chart is very large print, and normally sighted people can see it up to 60 meters away, but if you can only see it when close up sitting 6 meters from the chart, your vision is reduced to 6/60 in that eye. In other words 6 is the distance your are from the chart, and 60 is the lowest line you can read on the chart.



In America feet are used as the standard unit, rather than meters.Normal vision is considered 20/20, ie from a distance of 20 feet, normally sighted people can see the 20 feet line. 20/40 is equivalent to 6/12, 20/200 to 6/60 etc.

Self-Estimating Visual Acuity
Certain visual acuities have special significance. Some of these are:

• 20/20 vision is considered normal vision

• 20/40 vision in at least one eye is the vision required to pass the driving test

• 20/200 vision or worse is the legal definition of blindness

The chart below may be used by a customerto estimate his/her vision online.







Instructions:

1. Measure the blue bar in centimeters on the computer screen.

2. Step back to approximately the same number of feet from the computer screen. For example, if the blue bar measures 5 centimeters, the customer should stand 5 feet from the computer screen.

3. If the customer wears glasses or contacts to operate a motor vehicle, he/she should use them when taking the test.

4. Cover one eye and read the letters in row three (the third row from the top) from right to left. The vision testshould be performed on each eye separately.



The third line is equivalent to 20/40 vision. The customer must be able to read most of the letters on this line in order to obtain an unrestricted Massachusetts Driver's License. If he/she cannot read line three, the customer may be required to submit a vision screening certificate completed by a physicianor optometrist who is licensed to practice in Massachusetts. The vision screening certificate cannot be more than 12 months old from the date of the screening. A copy of the Vision Screening Certificate Form is available online here.



The middle letter of the top line is equivalent to 20/200. A person unable to read this letter with best eyeglass correction is considered legally blind. The "T" and the "B" on the top line are equivalent to 20/100. The second line is 20/50. Best corrected acuity in the range between 20/50 and 20/100 is considered disabling in occupations which require work with numbers or extensive reading.



The bottom three lines represent 20/30, 20/20 and 20/10. These are considered to be "normal vision" with the 20/20 line being the traditional normal vision line.



It is generally felt that the maximum visual acuity of the eye is around 20/15 or 20/16. This means that if the customer is able to read the bottom line, he/she is probably a good guesser.



Note that this vision test can only be considered a self-screening test, and is meant to give a customer an approximate idea of his/her vision capabilities. A variety of factors such as lighting, glare, monitor quality may have an effect on the results of this test. This does not constitute an RMV-approved vision test, and passing this test does not necessarily mean that a customer will pass the vision test at an RMV branch.
